Interim (6 month) Report for General
Support Grant or Project Grant
We understand that
unexpected challenges and opportunities influence the start-up or expansion of a
program. Therefore, we offer you an interim opportunity to share your
progress with us in a way that allows you to request or make necessary
adaptations as your learning unfolds.
In a letter format,
not to exceed three pages, please provide the following using a separate section
heading for each of the questions.

List and briefly
explain progress on the three goals you outlined in your preliminary
application.

List and briefly
describe obstacles/constraints to achieving the expected results. Likewise,
tell us of successes in moving forward.

Describe any
changes to your overall organization, board, as well as to project goals,
strategies, budget and/or personnel. How did you plan those changes?

Include your most
recent financial report showing organizational funds raised and expended to
date. If a project grant, indicate percentage of project budget raised and
project monies expended to date.

Include anything else you
would like us to know.
